Documentation Guidelines

Eligibility for services will be determined by one of the following means:

1. Assessment by a DSP&S professional staff
2. Review of documentation provided by appropriate agencies, certified or licensed professional outside of DSP&S;
3. In some instances, observation by DSP&S staff with review by the DSPS Director

DSP&S staff, with review by the DSPS Director, may verify a student's disability based on documentation provided by an appropriate agency or certified professional capable of diagnosing the disability in question or documentation from agencies participating in interagency agreements with the state Chancellor's Office.

Documents and Forms Needed

Learning Disabilities: require most recent IEP and Psycho-Educational Report (should be obtained from your high school)
Physical Disability: If you currently have a physical disability, please have your licensed practitioner complete and sign an Eligibility Verification Form and submit it to the DSP&S office.
Mental Health: If diagnosed with a psychological condition, please have your licensed practitioner complete and sign the Eligibility Verification Form and submit it to the DSP&S office.


To Return/ Received Documentation

  • All documents must be faxed (760-355-6434) or emailed (This email address is being protected from spambots. You need JavaScript enabled to view it.) to the DSP&S office.
  • Please title your fax or email "NEW STUDENT".
  • When your documents are received, they will be added to your file at DSP&S and will be submitted for review. Review process may take up to 14 working days.
  • If approved, you will receive an email indicating who will be your assigned counselor.
    PLEASE NOTE: If there is an increase on the applications received the review process may extend for up to four (4) weeks. It will be the student's responsibility to keep in mind the turn-around time when trying to meet semester deadlines.
